Anti-mesothelin vaccine CRS-207 with or without low-dose cyclophosphamide plus chemotherapy as front-line treatment for malignant pleural mesothelioma (MPM)

چکیده
Background CRS-207 is live-attenuated, double-deleted Listeria monocytogenes (LADD) engineered to express the tumor-associated antigen mesothelin which is highly expressed in malignant pleural mesothelioma (MPM). CRS-207 stimulates potent innate and adaptive immunity and in combination with chemotherapy may act synergistically to alter the tumor environment to be more susceptible to immune-mediated killing. Preliminary data of 32 patients who received CRS-207 in combination with pemetrexed/cisplatin showed 60% partial responses and 94% disease control[1]. Low-dose cyclophosphamide (Cy) in combination with LADD improved immune and anti-tumor responses and overall survival in preclinical studies.

Clinical experience with live-attenuated, double-deleted (LADD) listeria monocytogenes targeting mesothelin-expressing tumors
Immune-based cancer therapies have demonstrated benefit in multiple clinical trials. The interest in using recombinant bacteria as vaccine vectors for active cancer immunotherapy derives in part from their ability to stimulate multiple innate immune pathways and, at the same time, to effectively deliver antigen for presentation to the adaptive immune system. We have developed a vaccine platform...
متن کاملThe Economic Impact of Clinical Research in an Italian Public Hospital: The Malignant Pleural Mesothelioma Case Study
Background The current economic constraints cause hospital management to use the available public resources as rationally as possible. At the same time, there is the necessity to improve current scientific knowledge. This is even more relevant in the case of patients with malignant pleural mesothelioma (MPM), given the severity of the disease, its dismal prognosis, and the cost of chemotherapy ...

Methods In a Phase IIa randomized study, patients with metastatic pancreatic ductal adenocarcinoma (PDA) treated sequentially with two vaccine-based immunotherapies demonstrated extended overall survival (OS) with tolerable side effects. Treatment included low-dose cyclophosphamide (CY) followed by GVAX pancreas, an irradiated GM-CSF-secreting allogeneic PDA cell vaccine, and subsequent adminis...
متن کاملA prospective trial evaluating the role of mesothelin in undiagnosed pleural effusions.
Mesothelin has been proposed as a useful tool in the diagnosis of malignant pleural mesothelioma (MPM). We aimed to examine its diagnostic utility and the impact of renal impairment on results. We prospectively recruited 230 patients with new undiagnosed pleural effusions, testing serum (n=216) and pleural fluid (n=206) mesothelin (by ELISA) during the initial consultation. 28 (12%) out of 230 ...
متن کاملAdvances in the Medical Treatment of Malignant Mesothelioma
Malignant Pleural Mesothelioma (MPM) is a highly lethal and refractory malignancy that is caused by asbestos exposure. Surgical resection, radiotherapy, and other local treatments are of limited efficacy. Therefore, systemic chemotherapy plays an important role in improving of treatment outcomes for MPM. The findings of a largescale phase III study led to the approval of a novel antifolate, pem...
